What we collect
This site collects only what you choose to send us through the contact form: your name, a phone number or email address, and your message. That's the list. Like most websites, our hosting provider also receives basic technical information when pages load (such as browser type); we don't use it to identify you.
Why we collect it
One reason: so we can respond to you — usually by phone, sometimes by email. We may also use your message to prepare for your first call or visit. Nothing else.
Who we share it with
Almost no one. Your form message is delivered to us by Formspree, the service that powers our contact form — it processes your submission solely to deliver it to our inbox. Beyond that, we share personal information only if the law requires it. We never sell personal information, and we never share it with advertisers, marketers, data brokers, or other third parties for their own use.
Text messages (SMS) — Terms & Conditions
By opting in to SMS from our web forms, by phone, or in person, you agree to receive text messages from Meridian Neuro Rehab (Meridian Neuro, LLC). The kinds of messages you can expect: replies to your inquiries (conversational messages), appointment scheduling and reminders, and occasional updates about your care.
SMS consent is not shared with third parties or affiliates. Phone numbers collected for SMS are used only for these communications.

What we never do
We don't sell your information. We don't share it with advertisers or marketing companies. We don't add you to mailing lists you didn't ask for.
A note on health details
The contact form isn't a medical record, and you don't need to share medical details in it — a sentence or two about what's going on is plenty. Anything clinical is better discussed by phone. If you're having a medical emergency, call 911; this form isn't monitored around the clock.
Your choices
If you'd like us to delete a message you sent, or you have any question about this policy, ask us. Call (949) 850-2885 or email info@meridianneuro.net. A real person — the same people who answer the clinic phone — will handle it.
Changes
If this policy changes, we'll update this page and its effective date. We'll keep it in plain language.
