About Meridian

Two doctors. One focus. One way of working.

Meridian Neuro Rehab exists because concussion and dizziness recovery deserve a specialist's full attention — not a slot on a general PT schedule.

Why we do this

We kept meeting the same patient. Months past a concussion, or years into unexplained dizziness. Passed between providers, scans normal, told to rest, told it was stress, told to live with it. Not better. These are treatable problems — but they take time, precise testing, and a clinician who works with them every day. So we built a clinic around exactly that: a narrow focus, treated deeply, one patient at a time. No aides, no double-booked hours, no protocols off a shelf. If you're the patient we just described, you're who this place is for.

Dr. Nirav Patel, Doctor of Physical Therapy, at Meridian Neuro Rehab

Dr. Nirav Patel, DPT

Doctor of Physical Therapy — concussion and vestibular rehabilitation are his entire caseload.

Concussion and vestibular care reward depth, and depth is what Dr. Patel offers. When he maps your symptoms or tests your balance system, he's drawing on focused specialist training most physical therapists never pursue — applied to the conditions this practice was built around. That focus is why patients who've been everywhere end up here.

His patients tend to arrive after seeing several providers already. His job, as he sees it: be the last stop — the clinician who finally explains what's happening and walks the road back with you, one hour at a time. Ask him anything when you call; questions are welcome here.

Dr. Sapan Palkhiwala, Doctor of Physical Therapy, at Meridian Neuro Rehab

Dr. Sapan Palkhiwala, DPT

Doctor of Physical Therapy — concussion and vestibular rehabilitation are his entire caseload.

In practice, specialist depth shows up in the details: which eye-movement test to run, when the neck is the real driver, how hard to push an exertion session — and when not to. That judgment, built case by case in concussion and vestibular care, is what Dr. Palkhiwala brings to every hour.

He's known in the clinic for patience with complicated cases — the multi-system ones where symptoms overlap and previous treatment went nowhere. Those cases take longer to untangle. He thinks they're the reason to do this work at all.

How we work

Three commitments, every visit

Same doctor, every visit

No hand-offs, no rotating staff. The doctor who evaluated you treats you — and knows your whole story by heart.

A full hour, one-on-one

No aides, no overlapping patients. Sixty minutes of your doctor's full attention, in person or by video.

A plan built for your symptoms

Not a protocol off a shelf. One visit a week, with a plan that does its work between visits.

If screens are hard right now, you may have noticed this site is quiet on purpose — no motion, nothing flashing. It was designed to be easy on a recovering brain. So was our clinic.
