Why Meridian
You work directly with your doctor — not a rotating team of aides or a crowded treatment floor. Each visit is designed around your symptoms, your goals, and the plan that makes sense for your recovery.
Your visit is focused on you from start to finish — no hand-offs, no double-booking.
The practice focuses on concussion, vestibular, balance, and related neurological rehabilitation — for adults and teens.
You leave knowing what may be contributing to your symptoms, what happens next, and what you can do between visits.
Choose in-person care in Irvine, or telehealth across California when it fits your situation.

The doctors
Dr. Nirav Patel, DPT
Doctor of Physical Therapy — concussion and vestibular recovery, all day, every day.
Dr. Sapan Palkhiwala, DPT
Doctor of Physical Therapy — dizziness and post-concussion care are his focus.
You'll work with one of us — the same doctor, one-on-one, for the full hour, every visit.
How it starts
Call to schedule and ask us anything. If we're not a fit, we'll say so.
In person in Irvine or by video anywhere in California. A full one-on-one evaluation.
You leave knowing what's driving your symptoms, what we'll do, and what you'll do at home.
Fees & insurance
Some people need two visits a week, others one — we set frequency by what your recovery needs. Your plan works between visits, so you need fewer overall.
We provide a superbill you can submit for possible out-of-network reimbursement.
Pay with HSA or FSA funds, or any major card.
California lets you start physical therapy directly, without a physician referral. You can begin this week.
The honest answer: we're out-of-network, so coverage depends on your plan. Many plans reimburse part through a superbill. We'll walk it through with you before you spend anything.
Visit us
Available across California when it's appropriate for your care — we'll tell you honestly which fits.
Before you call
No. California's direct-access law lets you begin physical therapy without a referral — you can simply call us. If your case needs a physician's input, we'll say so and coordinate.
We're out-of-network, so we don't bill insurance directly. You pay us and receive a superbill to submit to your plan — many plans reimburse part of out-of-network care. HSA and FSA work too.
An itemized receipt with the codes insurance companies use. You submit it to your plan, and depending on your out-of-network benefits, they reimburse you directly for part of the cost.
Yes — across California. Much of concussion, vestibular, and speech care works well by video; we'll tell you honestly which parts of your care fit telehealth.
A full hour, one-on-one with your doctor. It's an evaluation: we look at what's driving your symptoms and you leave with a clear plan.
Lingering concussion symptoms, dizziness and vertigo, balance problems, and speech or thinking-and-communication changes — in adults and teens.
